These forms are intended to be used for online games. Here's how it works.

1. The GM tells the players which form they need to fill out and provides them with the URL for this page. The players use the Section 1 area below to find the form they need.

2. The GM also provide each player with a box name. The box name is created by the GM and can be any alphanumeric value. We advise GMs make it easy to remember because you'll need it later to retrieve the player's completed forms.

3. The player fills out the appropriate form, remembering as they do so to enter their given box name in the appropriate field.

4. The GM retrieves the completed forms by using Section 2 of this page. All they need to do is enter the box name they gave to their players.

NOTE: Seperate box names are advisable for security. That is, if you don't want the players to read each other's forms. On the other hand, having one single box name for all your players will allow them to see what the others are up to. We leave it up to the GM to decide which flavour of mandatory fun he prefers.

Also note that you can also request a form that is currently out of stock by using Section 3.
