Welcome to the Paranoia-Live Form Request Form.

This form will allow any GM to request their very own form for use on the Paranoia-Live Online Form Bureau. Here are some things to remember before requesting a form.

1. Give at least a weeks notice for the creation of a new form. We may be able to fulfil rush orders but there are no guarantees. Attempting to bribe one of our staff is of course treasonous and never to be attempted whilst the green light is showing on the close circuit camera in the corner.

2. Give a clear and full description of the form, including it's title, all question and answer text and the type of form objects included (drop down, radio buttons, etc). Vague requests for 'something a bit like so-and-so' may never get completed, unless agreement has been reached by one of our staff.

3. On completion you will be issued with the URL for the form so that you can keep it secret until your game. Unless otherwise requested, we will make it available for all at the Bureau a fortnight after the initial request.
