After playing In Nomine with a group of people from this site, this one's really a natural.
Warning: Religious content?
PARANOIA is Other People (I'm terrible with names.)
The forces of evil are very, very real. They lurk in the shadows, tempting us and driving us to sin and commit evil. What's worse is that our sins actually create more of them. Murder, Adultery, Insurance Fraud -- when these mortal sins are committed, monstrous demons are born.
Of course, the smaller sins make demons too. A married man's eye lingering just a few seconds too long on a passing woman, a kid eating the last cooking and forgetting to throw away the package, the driver who gets slightly annoyed at the worse-than-average-but-not-that-terrible traffic. Even these tiny acts, so inconsequential we wouldn't even think of them as sins, can cause beings of evil to come into the world.
Of course, tiny sins bring about tiny demons, and in Hell, size matters.
Players take the role of "imps," the most pathetic demons in existence. Most imps usually just get used as a snack by bigger demons, but occasionally a Demon Lord has a suicide mission or incredibly trivial task it can't afford to send valuable powerful demons on, and so it puts together a group of imps and sends them on their way.
Of course, the imps have to deal with several other threats besides the potential lethality of the mission, their own incompetence, the whims of their patron, bigger demons who want to eat them, and Angels who could literally destroy them just by looking at them.
They're all treacherous scum. Hey, what else could you expect from the lowest of the low of Hell?
See, each Demon Lord has their own agenda, and frequently passes along secret goals to imps on a mission. Sometimes, imps might even be working for The Enemy, who's offered them a chance at redemption, You-Know-Who knows why. Maybe they're a Humanist, and want to protect humans or keep both divine and infernal influence off of them, though considering the humans are much more in control of their own lives than the imps are, that's a little dumb. That there just abouts sums up how Zappy the concept is, I'm afraid. No attempt to maintain a character or their plot arc, only 2D reasons for betraying their fellow players.

I've always thought a game set in a desert, with a limited supply of water might work. Players have reasons to hate each other.

Unless...
Let say your team occupies a Church and finds a Priest, a free character. But that Priest doesn't automatically serve as an extra "life". You have to earn that extra life, by convicing the Priest to accept your secret society objectives. The Priest is free to align himself to whatever Player he wants. Once that Player kicks the bucket, the Priest can step in and serve as an extra "life".
Of course, your secret society can send you free "lives" in the form of allies. And of course, your fellow comrades can go off and kill those extra "lives" just to make sure nobody can take advantage of them. _________________

But that idea is only suited to certain types of secret mission - for example, it does not suit a mission along the lines of "don't let the team find out you've been bitten" because he hasn't.
That said, I can see some merit in a setting (hell, an entirely separate game) where the sole purpose is to recruit NPCs for use as replacement PCs - especially given that your fellow players will be trying to do the same.
